I have a debt on my credit report from over 7 years ago, another company bought it now it's showing up as a new debt with a different start date on it. Is there anything I can do to get rid of it?

I have really poor credit, how can I begin to fix it?
I have a credit score of 540, I have only one credit card with an $800 limit it's currently maxed, but paying it off this month, I have 13 months of perfect payments. I also have a judgement and several collections. I used the credit simulator and when I pay the credit card off it will go to 584. Will it increase if I pay off the judgements and collections even though they are almost 5 years old? Most of them are old medical bills.
