Why would a satisfied judgement be removed from Trans Union and not Equifax? Can I get it removed?
This was a medical bill but noted as a judgement/public record

It would depend on the date of the judgement/public record.  TransUnion is often more responsive then Equifax and will at times accept information provided by you while Equifax normally will not.   If it has been 7 years since the filing of the public record, obtain documents to show the date (IE: Court documents) and send a dispute to Equifax to have it removed.   Don't expect it to just disappear, Equifax will indeed contact the reporter of the judgement for validation.
