Which factor is effecting my credit report the most?
I have "very poor" credit

My TransUnion score is 610 and my Vantage score is 588

My credit card utilization is 77% (I understand 30% is a good place to be)

I have one item in collections for $1247 since 12/27/2011

My payments have been excellent and I don't have anything else on my report

Is my 77% credit card utilization effecting my credit score majorly or is it the collection I have since 2011?

The collections item is probably hurting you the most, because negative information does appear to have a very huge impact on your score. Clear up that account, pay down your current balances and continue to make payments each month.
