Tax leins first.

The IRS will work with you on a payment plan (they really aren't unreasonable) that will work.  A neighbor had threatening letters from IRS and he was able to get an arrangement that worked for him, and he has seasonal employment.Tax leins can be renewed.

But, you might be able to get help with the medical bills if the provider has a slliding scale payment program.  Might check that out.  And do a Google for Dave Ramsey for constructive ideas on how to get debt paid off without a whole lot of difficulty.
