My husband opened a credit card and added my name. How do I take my name off the card?
He got a offer from this company to get a zero interest loan to pay off whatever he wanted. So he took out $6000 and now this has brought my credit score down from excellent to good.

Call the Card issuer. If you were added as an "authorized user", you can unauthorize yourself at any time by calling the company and saying you don't authorize yourself to be on the card (doubly so if the balance is $0)
