I just became Guardian of an 87 & managing her finances. how do I get the documents to the bureaus
I want to notify all bureaus ...and I want to monitor all new & old action....I want to be alerted to any new charges that may occur.. basically freeze any open accounts...

Transunion, Equifax and Experian all have websites. There will be instructions on how to freeze credit. Monitoring credit is another thing, but you might not be able to or need to if the reports will be froze.
