How do I get Sams card off report when I am no longer authorized buyer for past 3 yrs?

You can dispute the account

You can do that at the website for the reporting agency that lists it on your report. Or you can write a letter to one of the Credit Bureaus and request its removal. 

You didn't mention how its being reported on your credit report.  Even closed accounts continue to be listed for 7 years on your credit report. So it may be showing as a closed account.  If its not you can notify the credit bureau that you no longer have access to the account.  

Of course,  If the account is in good standing and paid on time,  Having the account on your report is probably helping your overall score  and having it removed may lower your score in the short run.
