How can I have a POOR payment history when my payment score is 96.4%

96.4 % may look like a wonderfully high number, but the credit scoring models are extremely unforgiving in this area. If you go to the "credit factors" section here and read about how it's scored, you'll see the following breakdown:
100% Excellent

99% Good

98% Fair

97% Poor

96% Very Poor

On-time payment history is 35 % of your credit score and any late payment is going to have an adverse effect.  Work towards a 100% on-time history.
