The closed card from 2002 shoud have removed from your credit report in 2009.  The date will be depending upon the last payment made or the date the account was marked as closed.  Look at the date closely on the credit report.  If it has been 7 years, then write a letter to the credit burear asking that this item be removed based upon the agent of the account.  This is called a dispute.   You don't need a form letter, just a clearly stated letter of your request and why.   if it appears on multiple credit bureau reports, you should send the same letter to each of them.
